Website Usage and Limitations
No professional services are provided on or through this website. Telemedicine services may be provided, but only through a separate electronic health record (EHR) platform. There are no fees or services related to this website. This website is designed solely to relay information related to the Excelsis Practice. Any medical content should be considered informational only and does not constitute medical advice and should NOT be used to form any medical beliefs or make medical decisions. You should discuss any interesting information contained herein with your Provider(s).
Emergency Healthcare Situations
This website, any communications with the Practice, and face-to-face clinic assessments are NOT designed to or capable of handling emergencies. Call 911 or go to the Emergency Room (ER) if there is any concern for an emergency situation.
Messaging Notice
Patients should assume that any text, portal, or real-time messages WILL NOT BE SEEN in realtime, therefore patients should NOT RELY on non-verbal or written forms of communication and seek immediate medical attention for urgent matters (call 911).
Text/SMS
For your convenience, Excelsis Neurosurgery Dr. Phillips, using Spruce Health as its SMS text messaging service provider (only during business hours), can send you text message updates related to your care, such as appointment reminders, scheduling updates, and other administrative or care-related messages. Message frequency may vary, and standard message and data rates may apply based on your mobile plan.
You can opt out at any time by replying STOP, or get help by replying HELP.
After-hours & Response Times: Providers
Excelsis will diligently safeguard against any potential and foreseeable problems or complications related to your medical care. Providers of this practice will always be available, directly or indirectly, to Medical Facilities 24 hours a day. Therefore, hospital-based neurosurgery providers outside of this Practice may be involved in your care, especially for unexpected, unforeseeable, and/or emergency situations.
After-hours & Response Times: Patients
Response times for non-medical persons (patients/families) will be triaged, meaning that they will vary based on the degree of medical urgency. Please note that telephonic and/or online communications may not be adequate for assessment, so therefore patients should always consider face-to-face and hands-on assessment by an ER physician. We sometimes return calls in the evening and on weekends; please tell us when we should call you during those times.
Doctor-Patient Relationship
Trust and mutual respect are paramount to a successful doctor-patient relationship. No doctor-patient relationship exists until actual care is provided. There are circumstances where this relationship can be terminated. Patient non-compliance and disruptive behavior create justification for unilateral severance of the relationship. Failure to respond to office communications will be considered self of patient-initiated termination. If you have not been seen and/or have not had contact with our Practice for 1 year, you might be considered inactive. If you have not been seen in 2 years, we will consider you inactive. We would appreciate it if you inform us if you intend to seek care elsewhere so that we might share medical records.
